CO2 Laser for Med Spas: What to Know Before You Buy a Fractional System
A fractional CO2 laser is a different kind of purchase from an RF or light device. It ablates tissue, it is a prescription device, and it comes with room, safety and training decisions that a facial machine does not. This guide is for med spa owners and medical directors who are deciding whether a CO2 laser belongs on the menu and what to ask before they buy one.
We sell the SMAXEL fractional CO2 laser, so treat this as our view. We use the SMAXEL product page for specifications, FDA's own records for its clearance, and published reviews for the general science. We make no claims about results from SMAXEL, and we do not show device prices. Request pricing and we will send a quote.
The short version
- What it is. A fractional CO2 laser emits 10,600 nm light that water in skin absorbs. In fractional mode it treats columns of tissue and leaves skin between them untreated [S4][S5].
- The trade-off is recovery. Fractional ablative lasers keep much of the resurfacing power of older ablative lasers with less trauma, but published reviews still describe moderate downtime [S4].
- SMAXEL has a 510(k). FDA lists the SMAXEL CO2 Surgical Laser System as cleared under 510(k) K180036, for prescription use. The cleared indications are listed below [S2][S3].
- Cost is more than the machine. Plan for the payment, installation, training, room safety, supplies and service. The Med Spa Laser ROI Calculator helps you run the numbers with your own inputs.
How a fractional CO2 laser works
CO2 lasers emit light at 10,600 nm, which water absorbs strongly. Skin is mostly water, so the energy turns to heat at the point of absorption and vaporizes tissue water [S5]. One review notes that when the pulse is shorter than about a millisecond, the heat-affected zone is small, yet still enough to seal small dermal blood vessels [S5].
Fractional delivery changed how these lasers are used. Instead of treating the whole surface, the laser treats many small columns and leaves the tissue between them intact. According to one review, fractional ablative lasers, introduced around 2007, reduced treatment trauma and downtime while keeping resurfacing power. The same review still describes moderate downtime and moderate risk of complications for the category, and says these lasers remain at risk of scarring, discoloration and skin infection [S4].
On the SMAXEL page, the microbeam scanner delivers 100-micrometer columns of energy across a 2×2 to 20×20 mm field, with density from 9 to 4,489 dots and depth in five steps. It lists two fractional modes, Fractional and Low Pain Fractional [S1].
Fractional CO2 vs. erbium vs. RF microneedling
These three come up in almost every conversation about resurfacing and skin remodeling. The table summarizes what the cited sources say about each. It is not a head-to-head test of any IDS Lasers device.
| Fractional CO2 laser | Erbium (Er:YAG) laser | RF microneedling (RevaFlux) | |
|---|---|---|---|
| Energy | Laser light at 10,600 nm, absorbed by water [S5] | Laser light at 2,940 nm, also absorbed by water [S4][S5] | 2 MHz bipolar radio frequency delivered at needle depth. The RF can also be turned off for needling alone [S8] |
| What one review says about tissue effect | Vaporizes tissue water and leaves a thermal zone that can seal small vessels [S5] | Absorption about 16 times greater than CO2, with more precise ablation and less heat injury [S4] | Depth 0.0 to 2.0 mm, set by the clinician [S8] |
| Recovery, per the cited reviews | Moderate downtime for fractional ablative lasers [S4]. In a 2026 meta-analysis of acne scar studies, longer downtime and more pain than erbium-based lasers [S6] | Less postoperative swelling and fewer days of crusting than CO2 in one review [S4] | Not covered by the laser reviews we cite. Set expectations from your protocol and the IFU |
| FDA status of the IDS device | SMAXEL: 510(k) K180036 (see below) [S2][S3] | We do not sell an erbium laser | RevaFlux: FDA-cleared (510(k) K243097) for dermatologic electrocoagulation and hemostasis [S7] |
| In the IDS Lasers lineup | SMAXEL | Not in our lineup | RevaFlux |
Older reviews describe CO2 and erbium efficacy as similar, with some authors rating CO2 slightly higher, and call that point controversial [S4]. The 2026 meta-analysis pooled five small comparative studies (130 participants) on atrophic acne scars. It found a higher chance of better than 50% improvement with fractional CO2, along with more pain and longer downtime (downtime was pooled from two of the studies). When only randomized studies were counted, that efficacy difference was no longer statistically significant, and the authors call the evidence limited [S6]. Read it as context for the trade-off between response and recovery, not as a result you can promise.
If you are deciding between a laser and RF microneedling for your menu, our RF microneedling machine cost and ROI guide covers the RF side with the same cost framework.
Is SMAXEL FDA-cleared?
FDA's 510(k) database lists the SMAXEL CO2 Surgical Laser System under K180036: decision Substantially Equivalent, February 2, 2018, regulation 21 CFR 878.4810, Class II, Prescription use [S2][S3]. The indications for use in the clearance documents read:
- Non-fractional mode: incision, excision, ablation, vaporization and coagulation of human soft tissue, in dermatology, plastic and general surgery, gynecology, neurosurgery and podiatry.
- Fractional mode: ablative skin resurfacing, wrinkles, fine lines, rhytides and furrows, in dermatology, plastic and general surgery [S3].
SMAXEL is FDA-cleared (510(k) K180036) for those indications. Ask us for the instructions for use (IFU), and look up K180036 in FDA's 510(k) database yourself. Any CO2 laser vendor should be able to give you a 510(k) number and let you check it.

How to think about fractional CO2 laser machine cost
Device pricing depends on configuration and terms, so IDS Lasers quotes it directly. Request pricing. Financing is available through our financing partners. Here is what to put in your budget besides the machine:
- The payment. Use the monthly payment from your own quote, whether you finance or lease.
- Installation and training. Ask what is included and what it costs, in writing.
- Room and safety. Plan for protective eyewear and smoke control. Our professional line includes the DS-3000E smoke evacuator and the CRYO-ZET air cooling system, which our platforms page lists as built to sit beside SMAXEL [S9].
- Supplies and aftercare. The SMAXEL page lists no device consumables, but you will still have supplies per treatment. Ask for a written supply list. For aftercare, our shop sells The Laser Doctor post-laser serum: 20 ml for take-home ($18) and 500 ml for the treatment room ($125) [S10].
- Service and warranty. Ask what is covered, for how long, and what a replacement tube would cost. SMAXEL lists a glass CO2 tube [S1].
Then work out your break-even. The calculator uses this math: treatments per month to break even = monthly payment ÷ (price per treatment − consumable cost per treatment), rounded up. It leaves out labor, rent, card fees, marketing and taxes, so subtract those yourself [S7]. Choose SMAXEL in the device list and enter your own consumable cost. Only RevaFlux is pre-filled [S7].
